29 CFR 1910.37(b)(5): Each doorway or passage along an exit access that could be mistaken for an exit must be marked "Not an Exit" or similar designation or be identified by a sign indicating its actual use (e.g., closet). On/before February 13, 2024, in the following instances, doors directly leading to the outside of the building were not marked "Not an Exit". Employees were exposed to smoke and burn related injuries due to a potential fire hazard: a) Back Room: A fire door that led to the outside area where a walk-in cooler had been installed was unlabeled. This area is currently in the process of being fenced in and should not be considered an exit. b) Mechanic Shop: A double door that led directly outside to the parking lot was marked both "Exit" and "Not an Exit".

29 CFR 1910.157(e)(3): The employer shall assure that portable fire extinguishers are subjected to an annual maintenance check. Stored pressure extinguishers do not require an internal examination. The employer shall record the annual maintenance date and retain this record for one year after the last entry or the life of the shell, whichever is less. The record shall be available to the Assistant Secretary upon request. Bowling Alley Corridor between main entrance and exit: On/before February 13, 2024, Class A fire extinguishers were not subjected to annual maintenance checks. Employees were exposed to smoke and burn related injuries from the potential fire hazard.
