29 CFR 1910.333(a): Safety-related work practices were not employed to prevent electric shock or other injuries resulting from either direct or indirect electrical contacts, when work was performed near or on equipment or circuits which were or could be energized: a. On or about 02/16/2024, at 2725 Scherer Way North, St, Petersburg Florida 33716, an employee was using their hands to operate a 6-foot insulated work stick (Shotgun) to open a 7,200 volt switch gear, during which time the employer's work procedure did not require the employee to use adequate PPE/Personal Protective Equipment to protect the head, face, and neck; therefore exposing the employee to an arch flash hazard.

29 CFR 1910.335(a)(1)(i): Employees working in areas where there were potential electrical hazards were not provided with electrical protective equipment that was appropriate for the specific parts of the body that needed to be protected and for the work being performed: a. On or about 02/16/2024, at 2725 Scherer Way North, St, Petersburg Florida 33716, an employee was using their hand to operate a 6-foot insulated work stick (Shotgun) to open a 7,200-volt switch gear, during which time the employee was not wearing adequate PPE/Personal Protective Equipment to protect the head, face, and neck; therefore exposing the employee to an arch flash hazard.
