29 CFR 1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns.  Each employee working on a residential construction project exposed to possible danger of head injury from impact, or from falling or flying objects is not protected by protective helmets  The employer does not require the use of protective helmets when employees are exposed to overhead hazards and possible danger of head injury from impact, or from falling or flying objects during framing of a residential building.  This violation was most recently observed on or about February 21, 2024 at 7549 Peyton Parkway (Lot 4), Verona, WI  53593 where three employees were working and not protected by protective helmets.  To abate this violation, the employer must ensure that its employees are protected from head injuries from impact, or from falling or flying objects.  GL Construction of Madison LLC was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1926.100(a), which was contained in OSHA inspection number 1591017, citation number 1, item number 2 and was affirmed as a final order on August 28, 2023, with respect to a workplace located at 4671-4683 Bellflower Drive, De Forest, WI 53532.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed.  Ladders used for access to upper landing surfaces do not extend at least 3 feet above the upper landing.  The employer does not require that ladders extend at least three feet above the upper landing while framing a residential building.  This violation was most recently observed on or about February 21, 2024 at 7549 Peyton Parkway (Lot 4), Verona, WI 53593 where three employees were accessing the top of framed walls and trusses utilizing a portable step ladder that did not extend at least three feet above the upper landing.  To abate this violation, the employer must ensure that when ladders used to access an upper landing, the ladder side rails extend at least 3 feet above the upper landing surface being accessed.    GL Construction of Madison LLC was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1926.1053(b)(1), which was contained in OSHA inspection number 1526850, citation number 1, item number 2 and was affirmed as a final order on September 28, 2021, with respect to a workplace located at 2799 Endive Drive, Fitchburg, WI 53711.

29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b).  Each employee working on a residential construction project with unprotected sides and edges is not protected from falling by a guardrail system, safety net system, or personal fall arrest system.  The employer does not require the use of any form of fall protection when framing on a residential construction project when those activities take place 6 feet (1.8 m) or more above lower levels. This violation was most recently observed on or about February 21, 2024, at 7549 Peyton Parkway, Verona, WI, 53593 where three employees installing and bracing wood trusses of a residential building were exposed to falls of six feet or more to the lower levels because there was no fall protection used by the employees. To abate this violation, the employer must ensure that employees performing residential construction work are protected from falls of greater than 6 feet.
