29 CFR 1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level: On or about February 27, 2024, personal fall arrest systems were not rigged in a fashion that would prevent an employee from falling more than 6ft or contact any lower level. Personal fall arrest lanyards were observed being positioned below the employee within the second story of the residential building.
