29 CFR 1910.269(c)(1)(i):In assigning an employee or a group of employees to perform a job, the employer shall provide the employee in charge of the job with all available information that relates to the determination of existing characteristics and conditions required by paragraph (a)(4) of this section. On or about February 15, 2024, the employer did not ensure that an employee was provided all information in regards to an electrical job task assigned to them. An employee was tasked with the unblocking of an underground fed 3 phase 277/480V 16S meter. The employee was not provided information including but not limited to recognition of the meter as 3 phase 277/480v or the absence of a disconnect switch.

1910.269(l)(1)(iii): Electric lines and equipment shall be considered and treated as energized unless they have been deenergized in accordance with paragraph (d) or (m) of this section. On or about February 15, 2024 the employer did not ensure that electrical lines and equipment were treated as energized prior to work taking place. An employee was tasked with unblocking an underground fed 3 phase 277/480V 16S meter prior to the system being deenergized in accordance with 1910.269 (d) or (m).
