29 CFR 1910.22(a)(3): The employer did not ensure that walking-working surfaces are maintained free of hazards such as sharp or protruding objects, loose boards, corrosion, leaks, spills, snow, and ice. At the warehouse, employees were exposed to slip, trip and falls hazards in the following instances: 1) Oil spill and/or leak under a pallet Jax 500561 near the end of the charging isle. 2) Oil spill and/or leak under forklift with box on it, near end of the charging isle. 3) Water left on floor in Isle, next to shelving in cold storage area. 4) Water puddle and/or spill going into maintenance shop in the warehouse. 5) By the dry goods warehouse, unloaded pallets were stacked in the pedestrian walkways forcing the employees to walk into oncoming powered industrial trucks.

29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l): At the warehouse, employees engaged in material handling activities including but not limited to loading and offloading trucks using a rider pallet jack were exposed to struck-by hazards. The employer did not ensure that employee(s) had completed forklift operator training prior to permitting forklift operations.
