29 CFR  1926.416(e)(1): Worn or frayed electric cords or cables were used:  On or about March 6, 2024, employees engaged in residential construction activity at the job site located at 18769 Oakmont Drive, Omaha, Nebraska 68136 were exposed to electrical shock and burn hazards in that the  employees were using worn and frayed electric cords to power their tools.   Llamas Construction was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.416(e)(1), which was contained in OSHA inspection number 1585460 , citation number 1, item number 2 and was affirmed as a final order on 6-6-2022, with respect to a workplace located at 9817 south 184th Street, Gretna, Nebraska.

29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph  1926.501 (b):  On or about March 6, 2024, employees engaged in residential construction activities on the job site located at 18769 Oakmont Drive, Omaha, Nebraska 68136, were exposed to fall hazards in that the employer failed to ensure employees working greater than 6 feet were protected against fall hazards.  In this instance multiple employees were observed working at heights greater than 21ft without any form of fall protection.   Llamas Construction was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1646014, citation number 1, item number 1 and was affirmed as a final order on 4-17-2023, with respect to a workplace located at18405 Schofield Drive, Omaha, Nebraska.  Llamas Construction was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1585460, citation number 2, item number 1 and was affirmed as a final order on 6-6-2022, with respect to a workplace located at 9817 south 184th Street, Gretna, Nebraska.

29 CFR  1926.1052(c)(1)(ii): Stairways having four or more risers or rising more than 30 inches (76 cm), whichever is less, were not equipped with a stairrail system along each unprotected side or edge:  On or about March 6, 2024, employees engaged in residential construction activities on the job site located at 18769 Oakmont Drive, Omaha, Nebraska 68136, were exposed to fall hazards in that the employer failed to ensure stairways were guarded against fall hazards.  In this instance multiple employees were observed using a stairwell that was not equipped with guardrails, handrails or other form of fall protection system.   Llamas Construction was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.1052(c)(1)(ii), which was contained in OSHA inspection number 1585460, citation number 1, item number 5 and was affirmed as a final order on 6-6-2022, with respect to a workplace located at 9817 south 184th Street, Gretna, Nebraska.

On or about March 6, 2024, employees engaged in residential construction activities on the job site located at 18769 Oakmont Drive, Omaha, Nebraska 68136, were exposed to fall hazards in that the employer failed to document through certification the evaluation of employee training on the recognition of fall hazards on construction job sites; the proper use and inspection of fall protection equipment; and the proper use and construction of guardrail/stair-rail systems.
