29 CFR 1910.147(c)(7)(i)(A):Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation: On or about April 16, 2024, an employee was exposed to machine hazards associated with moving parts and in-going nip points during a new tool replacement in the mold section on Line # 7 extruder. The employer failed to provide training to ensure authorized employees understood the purpose and function of the energy control program and employees had acquired the knowledge and skills required for the safe application, usage, and removal of energy controls.

As a result, employee(s) working in the facility, and other forklift operators were exposed to struck-by and caught-in hazards. (b) On or about March 06, 2024, the employer failed to ensure that employee #2, who operates powered industrial trucks (PITs), such as, the Mitsubishi (model #FGC25N) (serial #AF82F45988), was evaluated on PIT performance at least once every three years. As a result, employee(s) working in the facility, and other forklift operators were exposed to struck-by and caught-in hazards.
