29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used. a. On or about March 19, 2024, at the above addressed jobsite, an employee performing masonry operations from tubular welded frame scaffold was not provided with safe access to the working platforms, sixteen (16) feet five (5) inches above ground, thereby exposing the employee to fall hazards.

29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms before being released for use by employees other than erection/dismantling crews: a. On or about March 19, 2024, at the above addressed jobsite, an employee performing masonry operations from a tubular welded frame scaffold, sixteen (16) feet five (5) inches above ground, was not protected by a guardrail system on all open sides and ends of platforms, thereby exposing the employee to fall hazards.
