In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports:   a) On February 18, 2026, employees were exposed to the hazard of falling, approximately 17 feet to the ground, while performing masonry work from a tubular welded frame scaffold platform that was not fully planked.   No abatement certification or documentation is required for this item.

29 CFR 1926.451(c)(1)(ii): Guys, ties, and/or braces were not installed  according to the scaffold manufacturer's recommendations or at the closest horizontal member to the 4:1 height and repeated vertically at locations of horizontal members every 20 feet (56.1 m) or less thereafter for scaffolds 3 feet (0.91 m) wide or less, and every 26 feet (7.9 m) or less thereafter for scaffolds greater than 3 feet (0.91m) wide:     a)  On February 18, 2026, employees were exposed to fall hazards while working from tubular welded frame scaffolding that was tied to the building where the height to the base width ratio of the scaffold was greater than 4:1.   No abatement certification or documentation is required for this item.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level was not protected from falling to that lower level:   a) On February 18, 2026, employees engaged in masonry work were not protected from the hazards of falling approximately 17 feet to the ground while working from a tubular welded frame scaffold platform.   A-Team Masonry, Inc., was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 126.451(g)(4)(i), which was contained in OSHA inspection number 1736607, citation number 1, item number 1b and was affirmed as a final order on February 27, 2025, with respect to a workplace located at Lot em07, Rosa Ln., Lemont, Illinois 60439.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.
