29 CFR 1910.29(f)(4): Openings in stair rails. The employer did not ensure that no opening in a stair rail system exceeds 19 inches (48 cm) at its least dimension. On or about March 26, 2024, Ensign Rig #773, TanMar Companies, LLC equipment exposed employees to trip and fall hazards to due employer not ensuring that no opening in a stair rail system exceeded 19 inches in the following instances: a) Two open staircases located in the front and back on Trailer MH0663. b) One open staircase located in the back of Trailer MH0973. TanMar Companies, LLC. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.29(f)(4), which was contained in OSHA inspection number1624253, citation number 1, item number 1 and was affirmed as a final order on December 12, 2022, with respect to a workplace located at Patterson Rig # 289 rig.

29 CFR 1910.305(g)(2)(iii): Flexible cords were not connected to devices and fittings so that tension would not be transmitted to joints or terminal screws: Ensign Rig #773: On or about March 26, 2024, outside TanMar Trailer #0973, a strain relief on a yellow/black (14AWG) extension cord was pulled away from plug. This exposed employees to electrical shock hazards. TanMar Companies, LLC. was previously cited for a violation of this occupational safety and health standard 29 CFR 1910.305(g)(2)(iii), which was contained in OSHA inspection number1624253, citation number 1, item number 2 and was affirmed as a final order on December 12, 2022, with respect to a workplace located at Patterson Rig # 289 rig.
