29 CFR 1926.1053(b)(1): Where portable ladders were used for access to upper landing surfaces, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladders were used to gain access.   a) 30 Milford Lane, Suffern NY:  Employees engaged in gutter securing activities on a flat pitch roof were exposed to fall hazards of approximately 20 feet to the asphalt below as a Werner extension ladder whose ladder side rails did not extend 3ft above the upper landing surface. Condition observed on or about 03/27/2024.  LRG Construction & Service Co Inc, Inc was previously cited for serious violations of this Occupational Safety and Health standard or its equivalent standard, 29 CFR 1926.1053(b)(1): which was contained in OSHA Inspection Number 1715192, Citation # 1, Item # 1, issued on 03/27/2024 with respect to a workplace located at 501 Lakeview Ave, Pompton Lakes NJ 07442. This citation became a final order on.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  a) 30 Milford Lane, Suffern NY:  Employees engaged in gutter securing activities on a flat pitch roof were exposed to fall hazards of approximately 20 feet to the asphalt below as a means of fall protection was not being utilized. Condition observed on or about 03/27/2024.  LRG Construction & Service Co Inc was previously cited for repeat violations of this Occupational Safety and Health standard or its equivalent standard, 29 CFR  501 (b)(13), which was contained in OSHA Inspection Number 1564537 and 1715192.  Regarding Inspection Number 1564537, Citation #01, Item # 001, was issued on 02/18/2022 to a workplace located at 37 Maplewood Blvd, Suffern NY 10901. This citation became a final order on 10/25/2023.  In regard to Inspection Number 1715192, Citation # 02, Item # 01, was issued on 03/27/2024 to a workplace located at 501 Lakeview Ave, Pompton Lakes NJ 07442.
