29 CFR 1910.119(f)(4):The employer did not develop and implement safe work practices to provide for the control of hazards during operations such as lockout/tagout; confined space entry; opening process equipment or piping; and control over entrance into a facility by maintenance, contractor, laboratory, or other support personnel: (a)Smithfield Foods, Inc. at 1400 North Weber Avenue, Sioux Falls, SD 57103: On or about and at times prior to March 27, 2024, the employer did not ensure that employees conducting line breaking activities on the Evaporator 0205, locked out and isolated all sources of anhydrous ammonia, prior to breaking the line. This condition exposed employees to anhydrous ammonia. (b)Smithfield Foods, Inc. at 1400 North Weber Avenue, Sioux Falls, SD 57103: On or about and at times prior to March 27, 2024, the employer did not ensure that employees conducting line breaking activities for Evaporator 0205 had verified the isolation of equipment containing anhydrous ammonia prior to breaking the line. This condition exposed employees to anhydrous ammonia. (c) Smithfield Foods, Inc. at 1400 North Weber Avenue, Sioux Falls, SD 57103: On or about and at times prior to March 27, 2024, the employer used Safety Relief Valve VF-101-0205 as a means for isolating anhydrous ammonia. The process safety information for Safety Relief Valve VF-101-0205 did not approve it's use as an isolating devices, exposing employees to anhydrous ammonia. (d)Smithfield Foods, Inc. at 1400 North Weber Avenue, Sioux Falls, SD 57103: On or about and at times prior to March 27, 2024, the employer did not follow their own line break policy in that the the individual who was responsible for assuring that the like break was conducted safely was not present when the line was opened.

This condition exposed employees to anhydrous ammonia. The following energy isolating devices required a lockout device: (1) Hot gas stop valve VH101-2080; (2) Solenoid valve VO103-0064; (3) MVP stop valve VL101-0205; (4) MVP solenoid stem valve; (5) Pump out valve VP101-2080; (6) Vent valve VV102-0205; (7) Safety relief valve (VF-101-0205)
