29 CFR 1910.147(d)(2): The machine or equipment was not turned off or shut down using the procedures established for the machine or equipment: a) Production department between Raw Line 3 and 4 - On or about January 30, 2025, the employer exposed employees to struck-by, caught-in and amputation hazards, in that the employer did not ensure the machine was shut down and energy sources were isolated when employees were required to place their hands in the danger zone of meat pump transfer pipes during the reassembling process.

29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees: a) Production department between Raw Line 3 and 4 - On or about January 30, 2025, the employer exposed employees to struck-by, caught-in and amputation hazards, in that the employer did not ensure the LO/TO devices had not been affixed to each energy source of the meat pump transfer pipes when employees were required to place their hands in the danger zone of meat pump transfer pipes during the reassembling process.
