29 CFR 1910.252(b)(2)(iii): Workers and other persons adjacent to the welding area were not required to wear appropriate goggles to protect them from the rays when noncombustible or flameproof screens or shields were not used: a) 5140 Martin Drive, Gap, PA 17527 - Small Parts Welding Department: On or about April 9, 2024, workers and other persons walking in the aisleways of the Small Parts Welding Department were exposed to arc welding rays from the lack of welding screens or requirement to wear appropriate goggles. Exposure to arc welding rays could result in eye damage and flash burns. b) 5140 Martin Drive, Gap, PA 17527 - Aluminum Welding Department: On or about April 9, 2024, workers and other persons walking in the aisleways of the Aluminum Welding Department were exposed to arc welding rays from the lack of welding screens or requirement to wear appropriate goggles. Exposure to arc welding rays could result in eye damage and flash burns. c) 5140 Martin Drive, Gap, PA 17527 - Single Frame Welding Department: On or about April 9, 2024, workers and other persons walking in the aisleways of the Single Frame Welding Department were exposed to arc welding rays from the lack of welding screens or requirement to wear appropriate goggles. Exposure to arc welding rays could result in eye damage and flash burns. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1910.134(c)(2)(ii): The employer did not establish and implement those elements of a written program necessary to ensure that any employee using a respirator voluntarily was medically able to use that respirator, and that the respirator was cleaned, stored, and maintained so that its use does not present a health hazard to the user: a) 5140 Martin Drive, Gap, PA 17527: On or about April 9, 2024, welding employees were voluntarily provided with powered-air purifying respirators (PAPR) and tight-fitting half-facepiece respirators and the employer did not establish and implement the voluntary use elements of a written respiratory protection program. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement.
