29 CFR 1910.184(d): Each day before being used, the sling and all fastenings and attachments were not being inspected for damage or defects by a competent person designated by the employer: (a) On or about April 10, 2024, the employer did not ensure that damaged slings were immediately removed from service, in that employees utilized frayed and damaged wire rope slings to a Chief Automotive Systems, Body Squaring and Alignment System, Model No. S-21M, Serial No. EZ95407, to lift and move vehicles as part of vehicle repair operations. This condition exposed employees working on the rig floor to struck-by hazards as a result of the wire rope slings failing.

29 CFR 1910.242(b): Compressed air used for cleaning purposes was not reduced to less than 30 p.s.i.: (a) On or about April 10, 2024, the employer did not ensure the compressed air nozzles used for cleaning off equipment in the facility were reduced to less than 30 PSI. The compressed air line was pressurized at approximately 60 PSI. This condition exposed employees using the air hoses to the hazard of an embolism in the blood stream and flying debris.

29 CFR 1910.334(a)(2)(ii): There was a defect or evidence of damage that could have exposed an employee to injury and the defective or damaged item was not removed from service: (a) On or about April 10, 2024, the employer did not remove from service an Chief Automotive Systems, Body Squaring and Alignment System, Model No. S-21M, Serial No. EZ95407, located in shop area, which had damaged wiring. The electrical cord connected to the pendant control had experienced substantial damage, exposing the wires below. This condition potentially exposed employees to injuries associated with electrical shock. Note: 1910.179(g)(1)(i): Wiring and equipment shall comply with subpart S of this part.
