1910.179(b)(6)(ii) Where passageways or walkways are provided obstructions shall not be placed so that safety of personnel will be jeopardized by movements of the crane. On or about April 4, 2024 at 140 North West Avenue, Youngstown, OH 44506, the employer failed to ensure employees utilizing a chain alloy sling to lift approximately 4000 pounds was kept free of crushing hazards in that a load fell on an employee when the load became free of the sling. The employer did not ensure that slings were securely attached to their loads.
