On or about April 4, 2024 at 140 North West Avenue, Youngstown, OH 44506, the employer failed to ensure employees utilizing a chain alloy sling to lift approximately 4000 pounds was kept free of crushing hazards in that a load fell on an employee when the load became free of the sling. The employer did not ensure that slings were securely attached to their loads.
