29 CFR 1926.602(c)(1)(vii): Unauthorized persons were allowed to ride on a powered industrial truck and a safe place to ride was not provided where riding of trucks is authorized: a) Worksite, corner of wooden roof truss, Genie GTH-844 Telehandler: On or about April 16, 2024, an employee was conducting framing activities while utilizing the forks on a Genie GTH-844 Telehandler as a work platform, at approximately 8 feet above the ground. The employee was not authorized to ride the telehandler was not provided with a safe place to ride and exposed to fall hazards. 29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee use appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors or potentially injurious light radiation: a) Worksite, roof: On or about April 16, 2024, an employee was conducting roofing activities while using a pneumatic nail gun without the use of eye protection.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: a) Worksite, roof: On or about April 16, 2024, employees were conducting roofing activities, approximately 15 feet above the ground, and were not protected from falling. The employees were wearing harnesses but were not attached to a lanyard or anchor point. A-Class Construction Corp was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1673079, citation number 1, item number 1, and was affirmed as a final order on December 15, 2023, with respect to a workplace located at 1522 North 4th Street, PHILADELPHIA, PA 19122.
