29 CFR 1926.451(f)(6): Scaffolds were erected, used, dismantled, altered, or moved such that they, or any conductive material handled on them, could have come closer than the minimum distances established for specific voltages as set forth in the table in subparagraph (f)(6).Note: Specify distances as indicated in table in paragraph (f)(6) of this section: a) Jobsite located at 2032 Reed St Philadelphia, PA: On or about August 22, 2024, the employer failed to ensure scaffold was erected to maintain the 10-foot minimum clearance distance from electrical distribution lines. Employees were performing bricklaying operations from a scaffold erected approximately 3 feet from electrical distribution lines, thereby exposing employees to electrical hazards.

29 CFR 1926.451(g)(1)(vii): For all scaffolds not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, each employee shall be protected by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section: a) Jobsite: 2032 Reed St, Philadelphia PA - On or about August 22, 2024, the employer did not ensure that employees conducting siding operations from a supported tubular welded frame scaffold on the 3rd level were protected from fall hazards by the use of a fall protection system or guardrail system, thereby exposing employees to fall hazards of approximately 19 feet.
