29 CFR 1904.7(b)(3): When an injury or illness involves one or more days away from work, the employer did not entry of the number of calendar days away from work in the number of days column, or if the employee is out for an extended period of time, the employer did not enter an estimate of the days that the employee will be away, and update the day count when the actual number of days is known: a) 4816 Zane A Miller Road, Waynesboro, PA 17268: On or about February 19, 2024, Column L for Case No. 24-1 on the employer's 2024 OSHA Form 300, Log of Work-Related Injuries and Illnesses, contained an "X" and did not contain the number of days away, an estimate of the number of days away, or a combination of the two. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

29 CFR 1910.132(d)(2): The employer did not verify, through a written certification, that the required workplace hazard assessment had been performed: a) 4816 Zane A Miller Road, Waynesboro, PA 17268: On or about April 17, 2024, employees utilized a Produtech ISEO fiber laser machine (A2 model) to laser cut stainless steel coil and the employees were using personal protective equipment (PPE), including, but not limited to, safety glasses, safety toed boots, and cut-resistant gloves. The employer did not prepare a written workplace PPE hazard assessment that outlined the hazards present which necessitated the use of the PPE. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

29 CFR 1910.1026(d)(1): The employer with a workplace or work operation covered by this standard did not determine the 8-hour time-weighted average exposure for each employee exposed to chromium (VI): a) 4816 Zane A Miller Road, Waynesboro, PA 17268: On or about April 17, 2024, employees utilized stainless steel coils containing approximately 16.5-19.5% of chromium on the coil laser (Produtech ISEO fiber laser machine, A2 model) and the employer had not determined the 8-hour time-weighted average (TWA) exposure for each employee exposed to chromium (VI). Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
