29 CFR  1910.215(b)(9):The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch:  In the maintenance shop, employees engaged in fabrication and grinding activities using bench grinders were exposed to struck by hazards in that the adjustable tongue guard opening was not maintained at a maximum of 1/4 inch in the following instances:   1. The opening from the abrasive wheel to the tongue guard was found to be 1/2 inch on the Norton Bench Grinder. 2.  The opening from the abrasive wheel to the tongue guard was found to be 1/2 inch on the Central Machinery Bench Grinder

29 CFR  1910.212(a)(1):One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  In the Flex Area, on machine 11, employee(s) hand feeding rubber coated material into the machine were exposed to an ingoing nip point at the spooler where the material is fed. The employer did not ensure the light curtain was  functional with the machine in operation.    HBD/Thermoid, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.212(a)(1), which was contained in OSHA inspection number 1533600, citation number 1, item number 1 and was affirmed as a final order on 08-27-2021, with respect to a workplace located at 1301 W. Sandusky St. Bellefontaine, OH 43311.    HBD/Thermoid, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.212(a)(1), which was contained in OSHA inspection number 1706288, citation number 1, item number 2 and was affirmed as a final order on 04-15-2024, with respect to a workplace located at 1301 W. Sandusky St. Bellefontaine, OH 43311.

If your establishment had 20 or more employees but fewer than 250 employees at any time during the previous calendar year, and your establishment is classified in an industry listed in appendix A to subpart E of this part, then you must electronically submit information from OSHA Form 300A Summary of Work-Related Injuries and Illnesses to OSHA or OSHA's designee.
