HBD Thermoid, Inc. on or about May 17, 2024, at the location of 1301 W. Sandusky Street, Bellefontaine, OH 43311: The employer did not ensure the powered horizontal drive shaft of the "Web Deduster" located on Extrusion Line One approximately 36" above the working surface was adequately guarded to protect employee from caught in hazards created by the rotating shaft. Employee tasked with manually adjusting the "Web Deduster" brushes with the hand wheel in proximity of the exposed powered horizontal drive shaft were exposed to caught in hazards.
