29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:  (a) At 11751 Huron St., Westminster, CO 80234O: On or about April 17, 2024, the employer did not ensure that employees were protected from caught-in and amputation hazards in that specific lock out/tag out procedures were not developed or utilized while working on energized equipment such as but not limited to the Safeline metal detector.  This condition exposed employees to caught-in and amputation hazards.
