Specific procedures shall be utilized during shift or personnel changes to ensure the continuity of lockout or tagout protection, including provision for the orderly transfer of lockout or tagout device protection between off-going and oncoming employees, to minimize exposure to hazards from the unexpected energization or start-up of the machine or equipment, or the release of stored energy; (a) At 11751 Huron St., Westminster, CO 80234O: On or about April 17, 2024, the employer did not ensure that employees were protected from caught-in and amputation hazards in that specific procedures were not utilized during shift and/or personnel changes as required on the Hamburg MF machine. This condition exposed employees to caught-in and amputation hazards.
