OSH ACT of 1970 Section (5)(a)(1):  The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were utilizing a modified electric chain hoist that exposed employees to struck-by hazards:     On or about April 17, 2024, in the east production rack-line area, employees were operating an electric chain hoist that had the following deficiencies:  (a) The hoist was modified in that a chain and sprocket were added so the apparatus would run horizontally instead of vertically. (b) The hoist was not labeled with the load capacity (c) The pendent controls not labeled.

On or about April 17, 2024. in the east production rack-line area employees were engaged in production operations in a high hazard area where the exit door swung inward, in the following areas:  (a) At the east Rack line bay on the north wall.  (b) At the east Rack line bay on the south wall.  (c) Facing North between east and west Rack Lines.  (d) Facing South between east and west Rack Lines.  (e) At the west Rack Line bay on the west wall.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws:   (a) On or about April 17, 2024, in east bay east Rack Line, the bushing had come away from the junction box providing power to the electric chain hoist, exposing the internal conductors of the cord and subjecting them to damage.
