29 CFR 1910.269(d)(2)(viii)(A): Retraining was not provided for all authorized and affected employees whenever there is a change in their job assignments, a change in machines, equipment, or processes that present a new hazard or whenever there is a change in the energy control procedures: On or about March 14, 2024, employees of Associated Electric Cooperation Inc. were testing in the neutral cabinet on Generator Unit 2 located at 41 St. Jude Industrial Park in Marston, Missouri. The employer had recently installed new monitoring equipment onto Generator 2 and did not train employees on the changes. The employer did not update the lockout/tagout procedures for the addition of the new monitoring equipment. An employee unaware of the power signal from the monitoring equipment was shocked when removing a lead on the current transformer.

29 CFR 1910.269(d)(6)(iii): All energy isolating devices that are needed to control the energy to the machine or equipment shall be physically located and operated in such a manner as to isolate the machine or equipment from energy sources: On or about March 14, 2024, employees of Associated Electric Cooperation Inc. were testing in the neutral cabinet on Generator Unit 2 located at 41 St. Jude Industrial Park in Marston, Missouri. The employer did not locate and isolate a 120V breaker providing power to a monitoring system recently added onto Generator Unit 2. The uncontrolled energy source unexpectedly shocked an employee when removing the ground lead on the current transformer.
