29 CFR 1910.119(f)(1): The employer did not develop and implement written operating procedures that provide clear instructions for safely conducting activities involved in each covered process consistent with the process safety information:  a.) United Refining Company, located at 15 Bradley Street, Warren PA, FCC Unit: On or about 3/5/24, prior to initiating a leak repair at the Everlast valve, the employer failed to implement FCC Unit Emergency Procedures when they failed to shut down the unit for emergency repairs. Employees suffered injuries including but not limited to burns and respiratory system irritation after in-process catalyst was released as employees loosened fittings.  ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below). This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement

United Refining Company, located at 15 Bradley Street, Warren PA: the employer failed to address the required personal protective clothing in written operating procedures including but not limited to, FCC Unit Emergency Procedures, Fry Panning Procedures, and Opening Line Procedures.  On March 5, 2024, employees suffered injuries including but not limited to burns and respiratory system irritation after in-process catalyst was released as employees loosened fittings.  ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below). On March 5, 2024, employees suffered injuries including but not limited to burns and respiratory system irritation after in-process catalyst was released as employees loosened fittings. 29 CFR 1910.119(f)(4): The employer did not develop and implement safe work practices to provide for the control of hazards during operations such as lockout/tagout; confined space entry; opening process equipment or piping; and control over entrance into a facility by maintenance, contractor, laboratory, or other support personnel:  a) United Refining Company, located at 15 Bradley Street, Warren PA, FCC Unit: On or about 3/5/24, prior to initiating a leak repair at the Everlast valve, the employer failed to implement the Fry Panning procedure Section 2.1, and Lockout/Tagout (Control of Hazardous Energy Sources) Section 4.3 Energy Isolation Procedure, when they did not prepare the equipment and develop an energy isolation plan prior to installing a fry pan on the Everlast valve and FCC Regenerated Standpipe.  Employees suffered injuries including but not limited to burns and respiratory system irritation after in-process catalyst was released as employees loosened fittings.  b) United Refining Company, located at 15 Bradley Street, Warren PA, FCC Unit: On or about 3/5/24 and times prior thereto, prior to initiating a leak repair at the Everlast valve, the employer failed implement the Line Breaking Procedure when they did not shutdown, isolate and depressurize the equipment associated with the Everlast valve and FCC Regenerated Standpipe in an orderly manner in accordance with FCC Unit Emergency Procedures. Employees suffered injuries including but not limited to burns and respiratory system irritation after in-process catalyst was released as employees loosened fittings.  c.) United Refining Company, located at 15 Bradley Street, Warren PA, FCC Unit: On or about 3/5/24 and times prior thereto, the employer failed to develop safe work practices for hazard evaluation, selection and use of personal protective equipment in the Line Breaking Procedure prior to a line breaking event for an attempted repair to the FCC Regenerator Standpipe.  Employees suffered injuries including but not limited to burns and respiratory system irritation after in-process catalyst was released as employees loosened fittings. PPE including but not limited to respiratory protection, FR gloves, and face shields were not utilized by the employees involved in the repair  d) United Refining Company, located at 15 Bradley Street, Warren PA, FCC Unit: On or about 3/5/24 and times prior thereto, the employer failed to develop safe work practices for hazard evaluation, selection and use of personal protective equipment in the Fry Panning Procedure.
