United Refining Company, located at 15 Bradley Street, Warren PA, FCC Unit: on or about May 9, 2024, the employer failed to implement their written Mechanical Integrity program Pressure Vessel Inspection Procedure, Ref. IS-06, (copy number 002069), Section 7.0 when they did not develop an inspection plan for inspecting the Fractionator (350-T-103). Failure to follow the written procedures could lead to releases of production related hydrocarbons and/or catalyst from the related equipment. b.) United Refining Company, located at 15 Bradley Street, Warren PA, FCC Unit: on or about May 9, 2024, the employer failed to implement their written Mechanical Integrity program Pressure Vessel Inspection Procedure, Ref. IS-06, (copy number 002069), Section 7.0 when they did not develop an inspection plan for inspecting the Regenerator (350-T-102). Failure to follow the written procedures could lead to releases of production related hydrocarbons and/or catalyst from the related equipment. c.) United Refining Company, located at 15 Bradley Street, Warren PA, FCC Unit: on or about May 9, 2024, the employer failed to implement their written Mechanical Integrity program Pressure Vessel Inspection Procedure, Ref. IS-06, (copy number 002069), Section 7.0 when they did not develop an inspection plan for inspecting the Reactor (350-R-101). United Refining Company, located at 15 Bradley Street, Warren PA, FCC Unit: On or about May 9, 2024, the employer did not establish written procedures for implementing the Management of Change requirements set forth in Procedure PSM-010 including but not limited to, establishing a necessary time period for temporary changes [1910.119(l)(2)(iv)], including the expiration dates and procedures for extending the temporary changes when needed. B.) United Refining Company, located at 15 Bradley Street, Warren PA, FCC Unit: On or about May 9, 2024, the failed to implement Management of Change procedure PSM-010, Section 4.0 when a request for change form was not completed within 48 hours of the implementation of the change.
