29 CFR 1910.119(f)(1): The employer did not develop and implement written operating procedures that provided clear instructions for safely conducting activities involved in each covered process consistent with the process safety information and addressing at least the following elements: Steps for each operating phase; operating limits; safety and health considerations; and, safety systems and their functions as outlined by this paragraph. a.) United Refining Company, located at 15 Bradley Street, Warren PA, Sat Gas Unit: on or about December 20, 2025 and at times prior thereto, employees conducted D-101 draining operations at least once per shift. Standard operating procedures had not been developed which included checking the D-101 liquid level, or the draining process for D-101. ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below). This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.
