The adjustable guard had been raised to accommodate a modification extending the sliding guide.

The outlet had inner conductors exposed and was providing electrical power to an extension cord and a scale. This condition exposed employees to electrical shock and burn hazards.

Exposure to formaldehyde can irritate the skin, throat, lungs, and eyes with repeated exposure to formaldehyde possibly leading to cancer.
