29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  Location: 2 Jackson Place, Framingham, MA - Roof  On June 2, 2024, workers were exposed to fall hazards of up to 25 feet to the ground below while installing roof shingles approximately 25 ft above ground level where fall protection was not used or was improperly used

29 CFR  1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed  Location: Jobsite 2 Jackson Place, Framingham, MA  On June 2, 2024, employees were exposed to falls from heights up to 25 feet to the ground while accessing an upper landing surface where an extension ladder was used to access a residential roof and the ladder side rails did not extend at least 3 feet above the edge of the roof.
