29 CFR  1903.19(c)(1): The employer did not certify to OSHA, within 10 calendar days after the abatement date, that the cited violation had been abated:  (a) Molondros Roofing, LLC, failed to certify abatement within 10 calendar days after the abatement date that each of the following cited violations had been corrected:  Citation 1, Item 1, a serious violation of 29 CFR 1926.1053(b)(7) with an abatement date of August 12, 2024.  Citation 3, Item 1, an other-than-serious violation of 29 CFR 1926.503(b)(1) with an abatement date of September 23, 2024.  In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERFICATIONS OF THE CORRECTIVE ACTION WORKSHEET).

29 CFR  1926.1053(b)(7): Ladder(s) that were unsecured or not provided with slip-resistant feet were used on slippery surfaces:  (a) On or about June 4, 2024, located at 636 Sunset Drive, Hartford, WI 53027, the employer did not ensure the ladder used to access the upper roof was secured or equipped with slip resistant feet to prevent incidental displacement on a slippery re-sheathed roof, exposing employees to a fall hazard of approximately eight feet.

29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  (a) On or about June 4, 2024, located at 636 Sunset Drive, Hartford, WI 53027, the employer failed to ensure employees tasked with installing plywood on a two-story residential home were protected by guardrail systems, safety net systems, and/or personal fall arrest systems, exposing the employees to a fall hazard of approximately 17.5 feet.  (b) On or about June 4, 2024, located at 636 Sunset Drive, Hartford, WI 53027, the employer failed to ensure employees tasked with passing sheets of plywood up to the two-story section were protected by guardrail systems, safety net systems, and/or personal fall arrest systems, exposing the employees to a fall hazard of approximately eight (8) feet.  Molondros Roofing, LLC. was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1631503, citation number 1, item number 1 and was affirmed as a final order on April 4, 2023, with respect to a workplace located at 311 E. Fernwood Lane, Appleton, WI 54913.  Molondros Roofing, LLC. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(1), which was contained in OSHA inspection number 1689809, citation number 2, item number 1 and was affirmed as a final order on December 7, 2023, with respect to a workplace located at 6220 University Ave, Middleton, WI 53562.
