Employees were not protected from fall hazards by guardrail systems, safety net system, or personal fall arrest system.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to upper landing surfaces, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladders were used to gain access. At the jobsite located at 5801 Newton Drive, Overland Park, Kansas; employees using the portable extension ladder to access the roof to perform roofing activities were exposed to fall hazards of over 9 feet from the roof to the ground in that the side rails did not extend at least 3 feet above the upper landing surface of the roof. The side rails extended approximately 2 feet.
