29 CFR 1910.147(f)(3)(ii)(D): Each authorized employee did not affix a personal lockout or tagout device to the group lockout device before working on the machine or equipment: On or about July 27, 2024, in the feta wet side, each employee did not affix a personal lockout device to a group lockout device (hasp) before they engaged in sanitation activities on the ATS-1 and 2 machines. This condition exposed employees to pinch point hazards.
