1926.21(b)(2) The employer did not instruct employees on the recognition and avoidance of unsafe conditions exposing employees to struck-by hazards:  (a)Runge Enterprises, Inc at 110 Pine Street, Yankton, SD 57078: On or about and at times prior to June 19, 2024, the employer did not ensure employees were protected from struck-by hazards created by rotation of the arm/attachment of excavators. One employee suffered serious injuries including facial fractures after being struck by a plate packer attachment on a Caterpillar 320 E excavator. Employees had not been instructed in the avoidance of unsafe conditions.   Among other methods the following are feasible and acceptable means of abating the hazards: Comply with instructions in the Caterpillar 320 E Excavator Operation and Maintenance Manual - Visibility Information (Page 36 - 38).
