29 CFR 1910.1200(f)(6): The employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged or marked with the information specified under paragraphs (f)(6)(i) through (ii) of this section: a) Worksite - in the Press 1 Room, 13985 S Eagle Valley Rd, Tyrone PA 16686, on or about June 21, 2024 - There were several plastic bottles that were not labeled with contents, and warning labels on the workstations. The containers were filled with a flammable solution, HS 2790 (80-20 Blend) which contained 1-Propanol and Propyl Acetate.

29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed: a) Worksite - in the Press 1 Room, 13985 S Eagle Valley Rd, Tyrone PA 16686, on or about June 21, 2024 - Press Room's (Press 1, Press 2, Press 3) Lockout/Tagout Posted Procedures were not inspected at least on an annual basis. The Press 1 Lockout/Tagout procedure stated revised 6/21/2021 with audit due Oct 2023 and next audit due Oct 2024. The annual audit was not completed in 2023.
