29 CFR  1910.176(b): Material stored in tiers was not stacked, blocked, interlocked or limited in height so that it was stable and secure against sliding and collapse:  Warehouse/ Turnings area - South wall. On or about June 26, 2024, Supersacks, IBC totes, 55-gallon drums, cardboard boxes, pallets, plastic totes, and other containers enclosing a variety of metal turnings, fluids, and other materials were stacked above six feet in height along walkways and exit accesses and were not stabilized or secured against sliding or collapse.

29 CFR  1910.157(c)(1): Portable fire extinguishers were not mounted, located and identified so that they were readily accessible without subjecting the employees to injuries:  Warehouse/ Turnings area. On or about June 26, 2024 the following conditions were observed: a) Southeast column. Two extinguishers (1-A and 1-ABC) were not mounted on or otherwise secured to a marked column, creating a potential hazard if moved or knocked over. b) West column - loading dock. An extinguisher was mounted within the webbing of a structural column, but was not identified in order to be readily accessible.

29 CFR  1910.22(a)(2): The employer did not ensure that the  floor of each workroom is maintained in a clean and, to the extent feasible, in a dry condition. The employer did not ensure that when wet processes are used, drainage is maintained and, to the extent feasible, dry standing places, such as false floors, platforms, and mats are provided.  Warehouse/ Turnings area - East wall. On or about June 26, 2024 the exit access floor was wet and covered with an unknown slick substance. Exit access was not possible without the risk of slipping.

29 CFR  1910.37(b)(4): Signs were not posted along the exit access indicating the direction of travel to the nearest exit and exit discharge when the direction of travel to the exit or exit discharge was not immediately apparent:  Warehouse/ Turnings area - Southeast wall. On or about June 26, 2024 the exit door located at the East end of the South wall was obstructed from view by IBC totes, Supersacks, and various other containers. No other signage was visible along the exit access.
