29 CFR 1910.132(d)(2): The employer did not verify that the required workplace hazard assessment has been performed through a written certification that identifies the workplace evaluated, the person certifying that the evaluation has been performed, the date(s) of the hazard assessment, and, which identifies the document as a certification of hazard assessment: In the manufacturing department at 79 Mainline Dr, Westfield, MA 01085: On or about June 27, 2024, employees wore personal protective equipment to include safety glasses, hearing protection and gloves, to protect themselves from exposures to hazardous chemicals such as wood dust, metal dust, aluminum oxide, Diethanolamine (CAS 111-42-2), Triethanolamine (CAS 102-71-6), and Beryllium Copper Alloys and no written certification of a hazard assessment had been completed.
