29 CFR  1904.41(a)(1)(i): The establishment had 20 or more employees but fewer than 250 employees during the previous calendar year, the establishment was classified in an industry listed in appendix A to subpart E of this part, and the employer did not electronically submit information from OSHA Form 300A Summary of Work-Related Injuries and Illnesses to OSHA or OSHA's designee by the specified date:  79 Mainline Drive, Westfield, MA:  As of April 23, 2025, the employer had not electronically submitted the information from their 2024 OSHA Form 300A. The employer's 2024 OSHA 300A from this establishment listed 62 employees and a North American Industry Classification System code of 332211 Cutlery and Flatware Manufacturing.
