On June 26, 2024, portable fire extinguishers were stored on the floor where they could be tripped over or knocked down. This condition exposed employee(s) to trip hazard(s) and/or not being able to readily access extinguishers to extinguish an incipient fire for escape.

29 CFR 1910.303(g)(1): Sufficient access and working space was not provided and maintained about all electric equipment (operating at 600 volts, nominal, or less to ground) to permit ready and safe operation and maintenance of such equipment: Station 2: On and/or before June 26, 2024, employees were working upstairs where an electrical panel was blocked by items such as vacuum cleaners and chairs. This condition interferes with access and immediate shutdown of a circuit in the panel, particularly in the event of an emergency.
