29 CFR 1910.178(l)(2)(ii): The employer did not ensure that each operator had successfully completed the training consisting of a combination of formal instruction (e.g., lecture, discussion, interactive computer learning, video tape, written material), practical training (demonstrations performed by the trainer and practical exercises performed by the trainee), and evaluation of the operator's performance in the workplace. (a) 3910 Oxford Road, Gettysburg PA, Shipping and Receiving work area - On or about July 3, 2024, the employer did not ensure that an employee had formalized instruction to safely use the walk/ride Barrett powered pallet jacks. The employee was exposed to struck-by hazards. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1910.178(l)(6):The employer did not certify that each operator has been trained and evaluated as required by this paragraph (l): (a) 3910 Oxford Road, Gettysburg PA, Shipping and Receiving work area - On or about July 11, 2024, the employer did not certify that employee(s) were provided operator training to safely use the walk/ride Yale and Barrett powered pallet jacks. Employees were exposed to struck-by hazards. Hillandale Farms was previously cited for a violation of this occupational safety and health standard 1910.178(l)(6), which was contained in OSHA inspection number 1726696, citation number 1, item number 4 and was affirmed as a final order on March 21, 2024, with respect to a workplace located at 1330 Austin Avenue Akron, OH 44306.
