29 CFR 1926.1053(b)(1): Where portable ladders were used for access to upper landing surfaces, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladders were used to gain access:  At the jobsite located at 22510 W. 60th Street, Shawnee Kansas, employees engaged in residential re-roofing activities were exposed to fall hazards of approximately 11 feet 7 inches from the roof to the ground below while accessing and exiting the roof in that the ladders did not extend at least 3 feet above the upper landing and were not secured to the residence/structure.
