29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury:  At the jobsite located at 21218 West 47th Terrace, Shawnee, Kansas, employees engaged in residential roofing activities with the use of pneumatic nail guns were exposed to struck-by hazards in that eye protection were not utilized.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):  At the jobsite located at 21218 W. 47th Terrace, Shawnee Kansas, employees engaged in residential roofing activities were exposed to fall hazards of approximately 10 feet 2 inches from the roof to the ground below in that fall protection was not utilized.

29 CFR 1926.20(b)(1): The employer did not initiate and maintain such programs as may be necessary to comply with the standards set forth in 29 CFR 1926 as evidenced by the violation(s) noted during the course of the inspection:  At the jobsite located at 21218 West 47th Terrace, Shawnee Kansas, employees engaged in residential roofing activities with the use of air compressor, pneumatic nail guns, ladders were exposed to struck by and fall hazards in that the employer had not developed nor implemented safety and health programs that provide construction hazard recognition guidelines to mitigate those hazards.

29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe condition(s) and the regulation(s) applicable to his work environment to control or eliminate any hazard(s) or other exposure to illness or injury:  At the jobsite located at 21218 West 47th Terrace, Shawnee Kansas, employees engaged in residential roofing activities with the use of air compressor, pneumatic nail guns, ladders were exposed to struck by and fall hazards in that the employees were not instructed in the recognition of construction hazards and the methods to mitigate those hazards. The employer did not provide hazard recognition training for employees performing roofing activities including, but not limited to how and when to use fall protection equipment, safety glasses, hard hats, how to set up ladder properly.
