29 CFR 1926.20(b)(2): Such programs shall provide for frequent and regular inspections of the job sites, materials, and equipment to be made by competent persons designated by the employers.  (a) On or about July 18, 2024, at a new construction residential jobsite located at 97 York St., Union Grove WI, the employer did not ensure the competent person was conducting frequent and regular inspections of the job site, to ensure hazardous conditions  were identified and corrected. Hazards identified included but were not limited to: fall protection not being worn, fall protection systems were not properly installed.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph         1926.501 (b):  (a) On or about July 18, 2024, at a new construction residential jobsite located at 97 York St. Union Grove, WI, employees conducting roofing work on a 6:12 pitch roof were not protected from falling to the lower level. This exposed employees to a fall hazard of approximately 25 feet onto dirt, gravel, and building material on the North and South sides of the structure and 11 feet, 7 inches  on the West side of structure.

29 CFR  1926.501(b)(14): Each employee working on, at, above, or near wall openings (including those with chutes attached) where the outside bottom edge of the wall opening is 6 feet or more above lower levels and the inside bottom edge of the wall opening.  (a) On or about July 18, 2024, at a new construction residential jobsite located at 97 York St. Union Grove, WI,  employees were standing at and sitting on an unprotected exterior wall opening (a window hole)  located on the  West side of the structure on the 2nd floor without fall protection or guardrail in place. This exposed employees to a fall hazard of approximately 11 feet, 8  inches onto dirt, gravel and building materials.

29 CFR  1926.503(b)(1):The employer did not verify the existence of fall protection training  by preparing and providing a written (training) certification record including the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer.  (a)On or about July 18, 2024, at a new construction residential jobsite located at 97 York St. Union Grove, WI,  the employer did not provide fall protection training to employees, to recognize fall hazards and implement fall protection methods while performing construction work above six  (6) feet in height.
