(a) On or about July 18, 2024, at a new construction residential jobsite located at 97 York St., Union Grove WI, the employer did not ensure the competent person was conducting frequent and regular inspections of the job site, to ensure hazardous conditions were identified and corrected. Hazards identified included but were not limited to: fall protection not being worn, fall protection systems were not properly installed.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): (a) On or about July 18, 2024, at a new construction residential jobsite located at 97 York St. Union Grove, WI, employees conducting roofing work on a 6:12 pitch roof were not protected from falling to the lower level. This exposed employees to a fall hazard of approximately 25 feet onto dirt, gravel, and building material on the North and South sides of the structure and 11 feet, 7 inches on the West side of structure.

(a) On or about July 18, 2024, at a new construction residential jobsite located at 97 York St. Union Grove, WI, employees were standing at and sitting on an unprotected exterior wall opening (a window hole) located on the West side of the structure on the 2nd floor without fall protection or guardrail in place. This exposed employees to a fall hazard of approximately 11 feet, 8 inches onto dirt, gravel and building materials.
