On or about July 24, 2024, near the intersection of 53rd and Cache Road, employees were performing pipeline work activities in an excavation and the side rails of the extension ladder the employees were using did not extend at least three(3) feet above the upper landing surface of the excavation.

29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(c).  On or about July 24, 2024, near the intersection of 53rd and Cache Road, employees were performing pipeline work activities in an excavation that was more than five(5) feet deep with unprotected walls.   W.E.B. Construction Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 29 CFR 1926.652(a)(1), which was contained in OSHA inspection numbers 1483288, citation number 1, item number 3, and was affirmed as a final order on September 14, 2020, with respect to a workplace hazard located at Corner of Britton and Sooner Road in Oklahoma City, OK.
