29 CFR 1926.651(k)(1): Daily inspections of excavations, the adjacent areas, and protective systems werenot made by a competent person for evidence of a situation that could result in possible cave-ins, indications of failure of protective systems, hazardous atmospheres, or other hazardous conditions. An inspection was not conducted by the competent person prior to the start of work and as needed throughout the shift. Inspections were also not made after every rainstorm or other hazard increasing occurrence. On or about March 10, 2025, at the north side of the worksite, employees were working in a excavation and a competent person had not inspected the excavation prior to the commencement of the work activities.

29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees. On or about March 10, 2025, at the north side of the worksite, employees did not have a safe means of egress when working in an excavation that was more than four (4) feet deep. W.E.B. Construction Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 29 CFR 1926.651(c)(2), which was contained in OSHA inspection numbers 1483288, citation number 1, item number 1a, and was affirmed as a final order on September 14, 2020, with a final abatement date of August 18, 2020, with respect to a workplace hazard located on the corner of Britton Road and Sooner in Oklahoma City, OK.

On or about March 10, 2025, at the north side of the worksite, employees were working within an excavation and the spoil piles were placed within two (2) feet of the excavation edge. W.E.B. On or about March 10, 2025, at the north side of the worksite, employees were performing pipeline installation work activities in an excavation that was more than five (5) feet in depth with unprotected sides and no protective system.
