29 CFR 1926.350(a)(7): A suitable cylinder truck, chain, or other steadying device was not used to keep compressed gas cylinder from being knocked over while in use: (a) Temporary welding tent, North side of parking garage: On or about July 24, 2024, and at times prior, the employer exposed employees to struck-by hazards, in that an acetylene cylinder was not secured by a truck, chain, or other steadying device, where employees were fabricating parts for a chiller installation.

29 CFR 1926.251(c)(1): Employer used wire rope in excess of the rated capacities (i.e., working load limits) prescribed by the manufacturer: a) Mechanical room, chain hoist: On or about July 24th, 2024, and at times prior, the employer exposed employees to crushing hazards, in that the steel cable used as safety lines to support a 10,000lb load had a safe working load (SWL) that was less than required while employees performed varied tasks required for the installation of a new chiller.

29 CFR 1926.251(c)(5)(i): The U section of U-bolt wire rope clips used to form eyes was not in contact with the dead end of the rope: a) Mechanical room, chain hoist: On or about July 24th, 2024, and at times prior, the employer exposed employees to crushing hazards, in that the U section of the U-bolts used to form an eye were installed with the U on the live end of the rope while employees worked in close proximity to a suspended load.
