The attachment point of the body harness was not located in the center of the wearer's back near shoulder level, or above the wearer's head. Location - On roof peak at the job site. On or about August 2, 2024, an employee was exposed to injury while working/walking on a commercial roof where the fall protection system used "waist belt" was inadequate to prevent injury.
