29 CFR 1910.28(b)(1)(i):Except as provided elsewhere in this section 29 CFR 1910.28, the employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling by one or more of the systems described in 29 CFR 1910.28(b)(1)(i): a.) U.S. Bronze Foundry and Machine, Inc., 18649 Brake Shoe Road, Meadville, PA 16335, on or about August 9th, 2024 and times prior thereto: Employees were exposed to fall hazards of 8 to 10-feet through a gap between the Centrifugal Spinner Wall and a concrete barrier while performing work activities around the Centrifugal Mold Spinner Pit. ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below). This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.

The employer did not make measurements which demonstrate the effectiveness of the mechanical ventilation system, such as capture velocity, duct velocity, or static pressure at least every 3 months, in controlling exposure.
