29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a.) Thermoforming, Threading Sheet into Chrome Rolls - On or about August 8, 2024, an employee sustained two fingertip amputations of the pinky and ring fingers, and a crushed middle finger of the left hand while threading a starter sheet in the chrome rollers of the Line 4 thermoforming machine. The employer did not ensure that an energy control procedure was developed and documented for the control of hazardous energy when employees are engaged in the service and maintenance activity of threading a sheet into the chrome rolls.
